An important drug dose error appeared in figure 6 of this Clinical Review by Mark S Cooper and Neil J L Gittoes (BMJ 2008;336:1298-302, 7 Jun, doi: 10.1136/bmj.39582.589433.BE). In the bottom centre box, a milligram unit should be a microgram unit. The start dose for vitamin D analogue (calcitriol or alfacalcidol) should therefore be 0.5-1 micrograms per day. An electronic problem caused the error.
