Thank you for this article, the first with practical advice for primary care.

The extremes are somewhat easy, self care or admit, but if the predicted crisis arrives when should we as GPs start community pneumonia antibiotics (amoxicillin /clarithromycin if not allergic, doxycline/clarithromycin if allergic in UK, cefalosporins in other countries)? And should these be part of a no admit palliative care package?
