What is the basis of Heidi Larson's confidence?

Dr Larson is apparently confident [1] but we do not know why? If vaccines are safe there should be a strong citable evidence base, but presently I am not sure which if any of the products on the routine schedule have been rigorously tested against saline placebo, and for all the targeted population.
