We congratulate the BMJ for identifying and presenting a very significant doctor-patient communication issue - one capable of causing unintended and unnecessary distress - in a format that not only debates the matter between experts but also provides a valuable patient commentary. Notwithstanding the well-articulated views of the three authors it is clear that for acceptable remedies to be found this debate needs to grow and metastasise widely, within both the profession and the population.
