A 61 year old man with schizophrenia presented with fever, fatigue, leg pain, and difficulty walking. He had rigid masses in his legs and elbows; the largest (14 cm diameter) in the left lateral malleolus had an open wound with discharge. Radiography identified a soft tissue mass at this site with multiple calcifications; the exudate had needle crystals, compatible with gouty tophi. His initial uric acid level was 494 µmol/L. The patient had decided to stop benzbromarone and potassium citrate prescribed for hyperuricaemia. Oral allopurinol, suppository diclofenac, and intravenous cefotiam hydrochloride were started after admission.
