Milk intake and risk of mortality and fractures in women and men: cohort studies

I have two questions regarding this article. 1) what about lactose free products? In a country like Sweden, where lactose free products are available so easily available, would this fracture and death rates lower if the people in the study drank lactose free milk? If so then it's not the milk per se that should be mentioned in the title but lactose ,right?
2) I'm also interested in how they had their fractures/deaths. Where they trying to reach something standing on a ladder for example or did they fall out walking ? Depending how the fracture occurred , the impact in the fall/injury can have a different outcome.
