Failures at four NHS organisations led to boy’s death from sepsis

A three year old boy who died an avoidable death was failed by all four NHS organisations involved in his care in the last days of his life, the parliamentary and health service ombudsman has found.1
Sam Morrish, of Newton Abbott in Devon, died of sepsis on 23 December 2010 after becoming unwell during a flu epidemic. The ombudsman, Julie Mellor, found that he would probably have survived had he not been let down by GPs, the local out of hours GP service, NHS Direct, and South Devon Healthcare NHS Foundation Trust.
