We agree that the diagnosis of cerebellar stroke or vestibular neuritis cannot be made solely on the patient’s ability to stand.1 The presence of sudden onset severe disequilibrium in patients with posterior circulation infarcts adds weight to the diagnosis, but the clinical context must be taken into account.
