Inquiry to be held into teenager’s death after radiation overdose

In this “In brief” News item (BMJ 2009;339:b3161, doi:10.1136/bmj.b3161), we wrongly edited the correctly written item. This resulted in the item seeming to suggest (by the phrase “given fatal radiation treatment”) that the radiation treatment had caused the girl’s death. This is not what we had intended to publish. The first sentence should have read: “A fatal accident inquiry is to be held into the death of a teenage patient with cancer who was given radiation treatment 58% higher than prescribed.”
