Rapid responses are electronic comments to the editor. They enable our users to debate issues raised in articles published on bmj.com. A rapid response is first posted online. If you need the URL (web address) of an individual response, simply click on the response headline and copy the URL from the browser window. A proportion of responses will, after editing, be published online and in the print journal as letters, which are indexed in PubMed. Rapid responses are not indexed in PubMed and they are not journal articles. The BMJ reserves the right to remove responses which are being wilfully misrepresented as published articles.
General Practitioners are being criticised for not reporting
incidents to the National Patient Safety Agency (NPSA).
I'm all for incident reporting and strongly agree that routinely
collected information on incidents and near misses can be used to learn
lessons and so reduce risk.
So - as we're all also reporting to our PCTs - why not get them to
routinely accept an NPSA report for their use too?
And if we're making changes, it'd also be nice if the form was a bit
more tailored to primary care. Or even a teeny bit supportive to the sort
of Significant Event Analysis we are already doing. As its GPs and nurses
(not specific admin staff) filling forms in primary care; I think the
whole thing could be shorter, easier, more relevant and - therefore -
filled in more often.
