With regard to the case reported by Aroori, Reddy, Jack and Jacob
(BMJ 328: 296), and with all due respect, the unusual feature is the
tongue lesions. To await temporal artery biopsy results before
instituting high-dose steroids, even if on short-term, speculative
grounds, but because of the central retinal artery occlusion, would seem
to be counter to common sense. Of course, the limited space available for
this case report in BMJ may also be a reason why some other relevant
details are omitted, and I shall be fully prepared to understand a more
detailed chain of events in this case.
