    As a trainee general practitioner in ophthalmology, I spent every Monday morning in the diabetes clinic. After five years in hospital medicine I had come to associate a fat stack of notes in young people with poorly controlled diabetes and the inevitable “you must do better or you will go blind/get renal failure/get angina/have a stroke” advice falling on deaf ears.
