Rapid responses are electronic comments to the editor. They enable our users
to debate issues raised in articles published on bmj.com. A rapid response
is first posted online. If you need the URL (web address) of an individual
response, simply click on the response headline and copy the URL from the
browser window. A proportion of responses will, after editing, be published
online and in the print journal as letters, which are indexed in PubMed.
Rapid responses are not indexed in PubMed and they are not journal articles.
The BMJ reserves the right to remove responses which are being
wilfully misrepresented as published articles or when it is brought to our
attention that a response spreads misinformation.
From March 2022, the word limit for rapid responses will be 600 words not
including references and author details. We will no longer post responses
that exceed this limit.

The Association of Physicians of India is to be commended for taking
the initiative in formulating guidelines for the management of
hypertension in India. This is unique, because it has taken into account
the special circumstances and needs of people in the Indian sub
continent.Hopefully this would set the pace for the development of other
guidelines for other conditions e.g. diabetes, whose prevalence is on the
increase.
As nutrition plays an important role in health, it would be appropriate to
have dietary guidelines specially formulated.
This is relevant because of variations in food supply, income levels,
population growth, life expectance, urbanisation etc, all of which can
affect dietary practices. Dietary guidelines based on the special needs
and ircumstances would provide nutrition information to consumers and
physicians for reducing the risk of other non commmucable diseases and
promoting health through rational dietary and healthstyle practices.
I read with great interest the formulation and issuance of guidelines for
the management of hypertension in the Indian population and I think was
long overdue.It ceratinly is a laudable achievement and such guidelines
may be issuedfor other conditions as well in India.
Some of the issues raised in the article as to the racial differences,
dietary habits, lower body surface area may have some relevance to the
management of the Asian patients in the UK and I would watch this space
with interest as to the response from the UK Cardiologists.
