    He had been a patient at a practice down the road for 10 years, but today he had chosen to reregister with me. Funnily enough, this was also the day that his sick note was due for signing. There was no acute problem, and, of course, we did not have his medical record. It was not an ordinary sick note that he wanted, but one which stated that he was incapable of work both now and in the future, so that he could send it off with some forms to his pension department.
