This is an exciting new service being set up with money from NHS England Community Fund. The model has been developed alongside our many partner agencies.

The area comprises 4 CCG areas – Vale of York; Harrogate & Rural District; Hambleton, Richmondshire & Whitby; Scarborough & Ryedale. The 4 CCGs have worked together with TEWV to create the proposal for this new service. The proposal has been awarded money from NHS England PMH Community Transformation Fund in wave 2.

The new service will operate across two TEWV localities (NY & Y&S) alongside the established Mental Health Services in each of the 4 CCG areas, which are centered on York, Harrogate, Northallerton & Scarborough. However, the service will be managed operationally by the York Directorate.

The model is for a single service running on a multi-hub model. Some staff (Medical/ Psychology/ Peer Support/ Nursery Nurse) will work across the county or into clusters of hubs. Care co-ordinators will work into one hub, based locally & sharing office space with local teams. This will foster good local relationships & ensure our practitioners are available & accessible in each area. Outpatient clinics & group work will be available in each locality, with frequency being flexible & depending on need.

The York hub is located at Huntington House, close to the city centre. The service will have hubs in the other 3 areas. Team meetings/ supervision etc will rotate through different venues to ensure it is accessible to all staff.

It is estimated that the total population of North Yorkshire & York is over 600,000. Annually there are about 7,500 births (VoY 3,600; HRD 1,600 ; HRW 1,400 ; SR 1,500). The service is commissioned to provide specialist Perinatal Mental Health (MHS) care to the 5% of these women who require the involvement of secondary MHS. The service is expected to see approximately 425 women per year. We will open to internal referrals in October 2018, with the team accepting referrals from outside MHS in January 2019. We expect just over 100 referrals per quarter at full capacity.

The new service is funded to allow 1.6 WTE consultant posts. An NHSE Perinatal Bursary Holder is in post. The two posts will provide clinical leadership to the team, deliver excellent PMH care. Liaison with partner agencies, within & outside MHS will be vital. Training & education will be a key role for the new service. In line with practice across TEWV, the Perinatal Mental Health Team (PMHT) will operate within TEWV’s innovative ‘Purposeful & Productive Community Teams model’:

The leadership cell will comprise consultant psychiatrists, team manager & consultant clinical psychologist. The team will meet in brief daily huddles to discuss clinical concerns, supported by regular team supervision, safeguarding supervision & reflective practice. This fosters close & supportive team-working. Daily huddles will be facilitated by technology to reduce travel whilst the other meetings are expected to benefit from personal attendance. The post holder will be a key part of the leadership team & work closely with other members of the PMHT & wider maternity services.

It is recognised that not every psychiatrist will have had the opportunity to have higher training experience within PMH. The successful applicant will be expected to work towards a credential in Perinatal Mental Health (due to be piloted shortly by the Royal College) and can be supervised by the Perinatal Psychiatrist in post. The credential will require regular formal supervision & some observed practice to be assessed.
