We have 2 posts, each for 6 months of ICM and ACCESS, commencing in August 2024.

You will spend 6 months working as a senior fellow at Newham University Hospital, where you will be a core member of the medical team, responsible for providing skilled care to critically unwell patients across the hospital, and you will spend 6 months working with London's Adult Critical Care Emergency Support Service, ACCESS, which provides for the transfer and retrieval of critical care patients across London.

Main duties of the job

ACCESS

• Join the dedicated, multi-disciplinary transfer team as the transfer doctor to

complete clinical, capacity and repatriation moves for critically unwell patients.

• Take clinical leadership of the team and undertake safe inter-hospital

transfers with distant supervision (after an appropriate period of supervision

and sign-off)

• Ensure thorough and accurate record keeping during transfers
• Perform effective clinical handover when handing over to receiving team
• Carry out appropriate equipment checks at the beginning and end of the day
• Attend daily pan-London briefings
• Carry out clinical transfers to the level appropriate to training
• Proactively engage in education, governance and QI activities on project days
• Have an active role in service development and training

Job description
Job responsibilities

ICU: As a member of the team the post holder will be responsible for the evaluation and management of patients referred to critical care as well as the provision of care to patients on the unit. The post holder will ensure that evidence-based care is delivered to the patients and will have responsibility for communication with other members of the multi-disciplinary team and with family members. The service is fully supported by the ICU consultant who has ultimate responsibility for the management of the patient. All significant decisions and plans, including decisions to admit, discharge and transfer, need to be discussed with the duty consultant and the outcome of these discussions need to be accurately documented in the patient record.

ACCESS: The senior fellow will be responsible for assessing patients suitability for transfer, working with the clinical teams and the ACCESS consultant to ensure that patients are able to receive the care they need. Following a successful assessment the Fellow will provide the clinical support during the transfer, delivering interventions as necessary to maintain the patients safety. At the receiving unit the fellow will handover the patient to the receiving team, and then work to ensure the team can green up for the next deployment.

Clinical Governance

The appointee will be expected to input into the Clinical Governance activities of the Trust to maintain the quality standards of clinical services. There is a monthly Governance meeting and the hospital holds a weekly Serious Incidents meeting. The post holder will be expected to actively participate in all aspects of governance activity to ensure that excellence of care will flourish.

Research and Development

The department has an active research focus, led by Dr Juan Martin. There have been four publications in the last two years along with poster presentations at international conferences. Newham has contributed hugely to the Recovery trial, recruiting over 200 of the 737 patients recruited by the whole Trust.

Time permitting, the post-holder will be encouraged to initiate and to participate in research programmes under the guidance of the critical care research lead, and the clinical lead in accordance with Trust policy. Extensive research facilities exist at Barts Health NHS Trust where collaborative and translational research will be actively encouraged. Research interests include Peri-Operative Optimisation, microcirculatory blood flow, acute kidney injury, genomics, Covid, VAP and critical care follow-up.

Teaching and training

The Trust is committed to sustaining and advancing the provision of medical, dental, nursing and other professional education and to the delivery of high quality postgraduate and undergraduate training.

The post-holder will be expected to participate fully in the following scheduled academic activities in Critical Care:

• Regular bedside teaching for the ICU Clinical Fellows
• Joint teaching with anaesthesia on alternate weeks
• Journal Club
• Weekly Critical Care teaching sessions
• Monthly morbidity and Mortality meetings
• Simulation sessions and other courses
• Yearly ICU session for the weekly Hospital Grand Round
• Induction and in-house teaching of Medical students from QMUL and overseas students and trainees.

Management

The appointee will be expected participate in wider service activities by attending monthly staff meetings and participating in M+M and QI.
Continuing Professional Development
The post-holder is expected to fully engage in appraisal process and will construct a personal development plan which will be reviewed on annual basis. The development plan will take account of general and specialist requirements for professional development issued by the relevant Royal Colleges, the General Medical Council, the Chief Medical Officer and the Trust itself. The post holder will be supported by appropriate study leave allocations with internal cover.

Critical incident reporting:

The post-holder will have responsibility for ensuring that critical incidents and near misses are appropriately reported into the Trusts Risk management systems.
