Job Details
The Bodmin CATU is a sub-acute frailty unit dedicated to providing a multidisciplinary team (MDT) approach for the comprehensive management of individuals living with frailty and related medical conditions. It offers a brief inpatient stay encompassing medical, nursing, occupational therapy, and physiotherapy assessments, followed by an early discharge to the patient's usual place of care. Post-discharge support is provided by Cornwall's Virtual Ward and community services.
The unit operates under nurse leadership with medical support. General Practitioners (GPs) & SAS Doctors collaborate with senior nurses on the ward, ensuring advanced medical decision-making for CATU inpatients. Referrals are accepted from various community providers, including the South Western Ambulance Service Trust (SWAST), the virtual ward, and GPs.
Main duties of the job
In collaboration with the senior nurses on the ward, provide advanced medical decision-making for CATU inpatients.
Offer medical guidance and leadership in the MDT management of patients.
Participate in advanced care planning (e.g., TEP) and end-of-life care.
Oversee and contribute to the education and training of junior doctors (including GP trainees), medical students, ANPs, and other AHPs.
Communicate with patients, families, and carers about their medical care.
Coordinate with other medical specialties, such as Eldercare and Palliative care, for specialist advice when necessary.
Engage in governance, addressing incidents, fostering learning, handling complaints, and other processes (e.g., legal) for the patients under your care. Assist the Clinical Director in these areas.
Establish and maintain connections with the Virtual Ward and the Same Day Emergency Care unit.
