Frailty Pathway Clinical Lead

An exciting opportunity has arisen to be part of the leadership team in supporting the Frailty Pathway for patients in Aberdeenshire and across the Primary and Secondary care interface. This is an opportunity to make a difference on a bigger scale. 

This is a flexible post which could work around an existing post or as part of a portfolio career. It will involve a mixture of Teams meetings and Emails. The hours for this post are 24 per week (3 days / 6 sessions).

The post will involve:

• Reviewing existing pathways and development of new pathways 
• Reviewing guidelines 
• Supporting our Virtual Community Wards 
• Delivering education 
• Quality improvement work 
• Involvement in local, regional and national groups and delivering on a wider strategy

Aberdeenshire Health and Social Care Partnership are looking for a dynamic GP to support and continually develop our Frailty and Pathways and the needs of patients, families and carers within community settings.
