The postholder will provide senior clinical support for the service at Ascot House which is a Discharge to Assess and Intermediate Care which Wythenshawe Hospital are commissioned to provide medical input to. This is a unit where older/frail patients are transferred following an acute illness for rehabilitation and reablement. The GP will provide medical management of patients and medical leadership of the team in conjunction with a Consultant Geriatrician.

The postholder will also have the opportunity to contribute to outpatient clinic activity and / or Hospital at Home work

  • Admissions, discharges,
  • Unwell pts
  • Speaking to families
  • Advance care planning
  • Respect discussions
  • Ongoing management of clinical issues following hospital discharge, as in the community,
  • Bloods/investigations as appropriate
  • Other ongoing referrals as required.
  • CGA
  • GP needs to have seen all the patients - (in case of death and paperwork)

Person Specification
Professional qualifications

  • Recognised Medical degree
  • MRCGP or equivalent
  • Full GMC Registration and License to Practice
  • Certificate of Completion of Training in General Practice
  • Inclusion or eligibility for inclusion on the Manchester Performers List


  • Relevant Higher Degree or management qualification/training


  • Broad experience in primary care and management of frailty / older people and / or with multi- morbidity including mental illness
  • Able to use EMIS systems


  • Leading of a multi- disciplinary team.
  • Primary and secondary care interface experience relevant to the patient cohort, e.g. GPwSI role


  • Commitment to and experience of undergraduate and postgraduate learning and teaching
  • Experience of involvement in a research project and publication or published audit project
  • Enthusiasm and commitment to the service
  • Ability to take the role of team leader
