Job description
Job responsibilities
The department is supported by an excellent clinical and management team providing a professional environment.
The laboratory underwent UKAS inspection in December 2021, after which we received a recommendation for accreditation. In May 2023 the department underwent follow up inspection and accreditation was maintained.
The department is in the final stages of implementing digital histopathology and consultant validation is expected to begin imminently. Consultant validation will be undertaken in stages in a stepwise manner and the new appointee will begin validation when appropriate for them and the department. This will be undertaken as per Royal College guidelines for pathologist validation.
Departmental Workload
Histopathology and non-gynaecological cytopathology requests at West Suffolk Hospital have steadily increased in the last few years with approximately 20000 surgical cases, 1500 non-gynaecological cytopathology cases and 350 adult coronial autopsies expected in 2023.
Like everywhere in the country case numbers have rebounded back after the COVID-19 pandemic. With almost all specialties across the trust instigating various catch-up clinics to address the NHS backlog, there has been a knock on increase the case numbers received into the department. In addition (and like the rest of the country) we have seen an increase in cancer work, specimen complexity and the volume of additional ancillary testing required (such as molecular work up).
Histopathology
The department is generalised and a range of diagnostic material is processed including a breast, skin, urology, gynaecology, gastro-enterology, thoracic, and a limited number of head and neck and thyroid specimens.
Frozen section requests have fallen in number in recent years, but facilities for providing such a service are present on site.
The laboratory is well equipped and provides a wide range of the commonly used immunohistochemical and special stains. Herceptin and Mismatch Repair (MMR) immunohistochemistry analysis is carried out in-house. Molecular testing is to be performed at the Genomic Lab Hub in Cambridge.
Cytopathology
The non-gynaecological cytopathology encompasses a range of material and has a particular major component of pleural fluids, bronchial washings, ascitic fluids, thyroid aspirates, urine specimens from haematuria clinic, and FNAs from Head and Neck. Clot preparations are made for all effusion samples to allow immunohistochemical analysis as required.
There is no requirement to report cervical cytology, but certainly any appointee could negotiate this as a specialist interest at the regional centre at the Norfolk and Norwich University Hospital
Autopsies
Currently we performing about 350 autopsies per annum, most of these being undertaken for HM Coroner. Coronial autopsies are not part of the NHS contract and are performed at the behest of the Coroner. Subject to the agreement of HM Coroner, the appointees are invited to share equally / pro-rata the coroners autopsies with the other consultants.
Hospital-based consented post-mortems are also performed, but are few in number. Autopsy work is a desirable and not an essential part of the post and is performed subject to the possession of the relevant RCPath training (or equivalent) and autopsy qualification. However, post-mortems have become an integral part of the departments teaching responsibilities.
The successful applicant will be expected to take part in all aspects of the work of the department.
1) Provide the Trusts histopathology and non-gynaecological cytopathology services, and autopsy services (subject to possession of the relevant experience / FRCPath autopsy qualification) in conjunction with the other consultant colleagues and the Pathology Departments scientific and technical staff. There is no requirement for the appointee to take part in the cervical cytology screening programme.
2) Pursue a special interest according to the needs of the Hospital and to take on the role of a sub-speciality Pathology Cancer Site Lead as needed by the department. This role includes developing cancer datasets and ensuring that all reports adhere to the agreed standards, liaising with clinicians in service planning and development, attending multi-disciplinary meetings, presenting colleagues cases where appropriate, and leading pathology aspects of audit projects.
3) Work closely with colleagues and the Cellular Pathology Laboratory Manager in the management of the service, to ensure a reliable and expeditious service and provide leadership where necessary in the continued development of the Histopathology service.
4) Participate in and present cases, including those reported by other pathologists, at clinical multidisciplinary team (MDT) meetings and grand rounds as appropriate.
5) Participate in undergraduate and postgraduate teaching. The Department has one histopathology specialist registrar who is assigned to a different consultant for supervision for 2 weeks on a rotational basis. The department also contributes to the teaching of post-graduate medical students from the Cambridge University Clinical School.
6) Take part in the relevant quality assurance schemes and proficiency testing as appropriate.
7) Join an appropriate CPD scheme, for which a standard amount of annual funding is available for study leave expenses.
8) Participation in Clinical Governance activities, including clinical and laboratory audit, and quality improvement programmes.
9) Deploy unallocated flexible professional time on liaison, emergency work, travel, clinical responsibility, teaching, research, medical audit, multidisciplinary meetings and continuing professional development.
