Gloucestershire Hospitals NHS Foundation Trust would like to invite applications for a substantive Specialist Doctor in Stroketo work mainly at Cheltenham General Hospital.

We are looking for a dynamic and enthusiastic colleague to join our well-established stroke service for the continuing delivery of high-quality patient care and to work towards ongoing service improvement.

We welcome applications from individuals with a minimum of 12 years' medical work (either continuous period or in aggregate) since obtaining a primary medical qualification of which a minimum of six years should have been in a relevant specialty in the Specialty Doctor and/or closed SAS grades. Equivalent years' experience in a relevant specialty from other medical grades including from overseas will also be accepted

We are looking for a highly motivated clinician able to work within a team of consultants, nurses and allied health professionals.
Main duties of the job

  • Maintenance of high quality patient centred care
  • Shared responsibility for the day to day management of the service and participation in strategic planning
  • Teaching and training within the multidisciplinary service
  • Active involvement in multidisciplinary team meetings and directorate clinical governance activity
  • Active participation in continuing medical education
About us

Stroke and TIA services have been reorganised following the Covid crisis and comprise a direct admission pathway via triage conversation with paramedics and GPs direct to CT avoiding ED which has been invaluable in bed crises to maintain the stroke pathway, a 8-10 bed HASU currently at Cheltenham General Hospital, with, then onwards to an acute (combined acute/rehab) unit also at CGH (ASU- Woodmancote ward 32 beds) and a subsequent 14 bed rehab unit at the Vale community Hospital, Dursley. We have a well-developed stroke Early Supported Discharge team covering

the whole county (consisting of Physio, OT, SALT, Psychology, Nurse), and Community Stroke Nurses support patients in the community and coordinate their 6 month reviews. Our tertiary stroke centre for mechanical thrombectomy and neurosurgery is Southmead Hospital (NBT), Bristol.

The Stroke Thrombolysis rota is shared with the other Stroke Consultants in this network and provides telephone support with online viewing of scans, but no need to return to site.

We are also part of the South West Stroke Research Network with 2 Stroke Research nurses, and there are many opportunities for clinical research and participation in multicentre studies.

The Stroke Service has close links with the Neurology team (currently Consultant neurologists based at Gloucestershire Royal Hospital). There is scope for the development of a spasticity service for stroke patients.

Job description
Job responsibilities

  • To be responsible for 1 in 6 weekend ward rounds (8am to 2pm) (no on call)
  • To potentially participate in the regional thrombolysis network rota for telephone advice and teleradiology (1 in 10-12)
  • To be the HASU 1 consultant of the week (Monday to Friday 8am to 4 pm). Usually 1 in 5. This includes a thrombolysis and thrombectomy assessments and ward referrals in conjunction with stroke specialist nurses and other back up HASU consultants (HASU2)
  • To act as the HASU 2 consultants usually 1:5. This involves support to the stroke specialist nurses and HASU 1 consultant for referrals and front door assessments and TIA clinics (telephone and face to face)- Monday to Friday.
  • To be the ASU consultant of the week (usually in a 2-3 week block) on Woodmancote ward, Cheltenham General Hospital
  • To visit the Vale Community Hospital (Rehab SU) - infrequently (typically once in 6 weeks) for an advisory ward round for the GPs who have daily oversight of the patients there.
  • To be responsible for providing advice and input to stroke patients referred from other specialties (usually during HASU1/2 blocks)
  • To provide outpatient clinics new strokes and follow up (1 clinic per week)
  • There is scope for development of a spasticity service and to participate in research multicenter trials and local research project

Qualifications
Essential
  • Full General Medical Council registration
  • MRCP or equivalent experience
  • Shall have completed a minimum of 12 years' medical work (either continuous period or in aggregate) since obtaining a primary medical qualification of which a minimum of six years should have been in a relevant specialty in the Specialty Doctor and/or closed SAS grades. Equivalent years' experience in a relevant specialty from other medical grades including from overseas will also be accepted
Desirable
  • Higher degree
  • Additional qualification in Quality Improvement, Medical Education, or other pertinent expertise
Clinical Experience
Essential
  • Proven clinical skills in Stroke medicine including thrombolysis and/or thrombectomy assessments
  • Ability to lead a multi-disciplinary team
  • Ability to work closely with colleagues in Acute Medicine and Emergency Medicine
Desirable
  • Experience in Medical Education and/or Research
Management & Administrative
Essential
  • Participation in clinical audit including completion of audit project
  • Understanding and experience of Clinical Governance and Quality Improvement
  • Willing and able to work with colleagues in other specialities, professions and roles to ensure the delivery of a high quality, safe and reliable service
  • A willingness to accept shared responsibility and provide leadership
  • Awareness of current relevant developments within the wider NHS
  • Determination to have a key role in establishing and developing the service
