An opportunity has arisen for an addition Gastroenterologist to join the West Suffolk Hospital Gastroenterology department.

This post has been created to help the department further reduce waiting times within both Clinic and Endoscopy and the job plan will be focused on supporting these areas. You will be working within a high performing team, supporting outpatients and inpatients receive the best possible care and outcomes.

The role has a flexible element to it as well enabling the successful candidate to work more flexibly.

Main duties of the job

Key tasks:

Maintenance of the highest clinical standards in the management of all patients under your care

To work with the Gastroenterology team in achieving access targets for the outpatient and endoscopy service.

To participate in the management of patients referred under the two week wait system for suspected gastrointestinal and hepato-pancreatico-biliary cancer and participate in the associated multi-disciplinary team meetings.

Maintenance of the highest clinical and technical standards, when undertaking scoping procedures, participating in the management of all patients presenting to the Endoscopy Unit.

To share with colleagues the responsibility for the management of the Gastroenterology Department and Endoscopy Unit.

Teaching and training of intermediate, junior staff, nursing staff and medical students

To actively participate in both departmental and Trust matters concerning Clinical Governance and audit.

To have responsibility for ensuring active participation in continuing professional development (CPD).

Post will be 10 PAs (can go up to addition 1.25PA as flexible activities with the agreement of management)

DUTIES OF THE POST

Full Time Consultant in Gastroenterology and Endoscopy Applications are invited for this substantive post of Consultant in Gastroenterology and Endoscopy at the West Suffolk NHS Foundation Trust. This post comprises 6 fixed sessions: three outpatient and three Endoscopy sessions. There is no General Medical component to this post.

Duties & responsibilities of the post The appointee will be expected to provide a clinical and administrative service in Gastroenterology and Endoscopy.

Endoscopy: The post holder will be expected to perform or supervise the following endoscopic examinations:

Oesophago-gastro-duodenoscopy

Flexible sigmoidoscopy

Colonoscopy

The post holder will perform both diagnostic and therapeutic procedures in the above areas doing 3 endoscopy sessions a week.

In addition to the performance of the actual procedure, the post holder will perform or oversee all the duties allied to the procedure, e.g. consent, administration of sedation, reporting, reviewing patients as may be required pre-discharge, and communicating with other health professionals as may be required in the interest of managing the patient. Leadership of the clinical team, including nursing staff, assistants, and other medical staff will be required, with the post holder assuming responsibility for the clinical care and safety of the patient. The post holder will participate in a 1 in 8 out of hours emergency bleeding rota.

Gastroenterology:

The appointee will join a department, which includes Consultant Gastroenterology colleagues Dr Dan Sharpstone, Dr Tyara Banerjee, Dr Fort Gasia and Dr Dan Gavin who have a special interest in inflammatory bowel disease, Dr Simon Whalley, Dr Abdullah Mawas and Dr Nadim Sheikh, who have a specialist interest in liver disease and Dr Krishna Basavaraju who has a specialist interest in nutrition. 8 The creation of this new post is intended to meet requirements of waiting list targets, increased clinical activity and improving the quality and timely provision of specialist Gastroenterology service in this busy hospital department. This post will comprise 3 Endoscopy sessions and 3 outpatient gastroenterology sessions a week. The outpatient sessions will be a combination of face to face, telephone and specialist gastroenterology clinics tailored to the successful candidate.

The post holder will also be expected to assist with the review and assessment of gastroenterology inpatients as required and at time be expected to manage Gastroenterology or General Medical inpatients.

Administrative: To produce, maintain and update departmental clinical management guidelines in line with evidence based best practice on a continuing basis. The appointee will be encouraged to take an active role in any relevant Trust committees or sub- committees and any local or regional Gastroenterology meetings.

Educational Arrangements: The University of Cambridge Medical School Graduate Course is based at the West Suffolk Hospital, which led to the development of a new teaching complex. The appointee is expected to participate in teaching and training of medical students on the Graduate Course and the traditional Cambridge course, junior staff and other clinical staff groups. The appointee will also have supervision responsibilities for junior medical staff within the specialty. Supervision of the training of Junior Doctors in the Department will be required in accordance with their provisions of the Royal College of Physicians. Contribution to grand round education through the Education Centre is encouraged. If appropriate, the post-holder will be named in the contract of junior staff as the person responsible for overseeing their training and as an initial source of advice to such doctors regarding their careers.

Timetable: The job plan is 10 PAs which includes 3PAs for 3 clinics, of which at least two will be telephone 3PAs for 3 endoscopy sessions, of which at least two will be flexible activity (84 sessions delivered across the year) 1.5 SPAs for CPD 2.5 PAs for admin, ward referrals and MDT. 9 This can go up to additional 1.25 PA to deliver 42 flexible activities within 1 year, subject to agreement between the post holder and the Service Manager for Gastroenterology. The number of patients seen per OP clinic amounts to 15-18 units (Outreach clinics 15 units), where a follow up is 1 unit and a new patient is 2 units. Endoscopy lists are 10-12 points.

On Call & Emergency Duties for Gastroenterology: The post holder is expected to take part on 1:8 out of hours endoscopy bleeding rota. 3.5 Annual Leave Arrangements and Cover The Consultants of the Department of Gastroenterology generally take their annual/study leave in conjunction with colleagues to ensure continuity of consultant care. Senior annual leave should not conflict with other colleagues. Leave must be discussed and agreed with departmental colleagues before being signed off by the directorate managers. Please note that Trust policy is that six weeks notice is required for leave. Prolonged periods of leave or absence beyond four weeks must be discussed with the Clinical Director. Locum cover may then be granted in line with existing Trust policies and directives at the discretion of the Clinical Director. Any intended deviation from these arrangements must be discussed with, and approved in advance by, the Clinical Director.

Job Plan: A job plan will be agreed by the Clinical Director and General Manager in the first instance and approved by the Medical Director on behalf of the Chief Executive. This plan will be reviewed annually and will provide an opportunity for discussion of the individuals personal development plan and for appraisal. The annual review will be used to check that the record of fixed and flexible sessions is accurate and compliant with the EWTD. This also provides an opportunity for constructive debate about the individuals personal development plan including CPD and any evolving special interests, which the appointee may wish to pursue. Besides the fixed clinical sessions necessary to provide continuous cover, flexible sessions will be agreed to include time for teaching, administration, management and Clinical Governance. Further SPAs may be available on negotiation with the Clinical Director and Associate Director of Operations for additional duties such as clinical governance activities and educational supervision of juniors. 10 Clinical Governance/Audit is undertaken as an ongoing programme within the Region. It is expected that the appointee will be actively involved in this and all routine clinical duties should be suspended during these sessions

Mentoring: The new appointee will be supported and mentored for the first six months of the post it will be provided by one of the consultants in the department, depending on the appointees choice and by arrangement.

  • Clinical training and experience equivalent to that required for gaining UK CCT in Gastroenterology
  • Ability to take full and independent responsibility for clinical care of patients
  • Experience in management of Gastroenterology in- patient and out-patient medicine
  • Ability to offer expert clinical opinion on a full spectrum of Gastroenterology patients
Desirable
  • Five years supervised training in an appropriate StR training programme or equivalent.
  • Training in a sub- specialty of Gastroenterology complementary to that of the existing consultants.
Qualifications

  • Full GMC registration
  • Entry on the GMC specialist register in Gastroenterology or be within six months of attaining Certificate of Completion of Specialist Training at the time of interview
