Specialist GP, Oxford City Enhanced Care Virtual Ward

This is a unique opportunity to join a cutting-edge project to provide joined up and enhanced care to older people in Oxford. Older people who are medically unwell and at risk of hospitalisation will be supported by a ‘Virtual Ward’ in the community. The service provides both a stepup from the community to avoid unnecessary admissions, as well as step down from secondary care to reduce length of stay. It will complement the work of ‘acute Virtual Wards’.

The ECVW team includes dedicated GPs, physicians associates, paramedics and support workers, working with the patient’s usual GP practices. This means that the service can support patients with higher acuity and greater complexity. The service is developing and builds on the successful Primary Care Visiting Service run by a collaboration of Oxford City Primary Care Networks (Oxford City Primary Care).

We are looking for 2 or 3 Specialist GPs for a minimum of 2 sessions a week each, and ideally more. The successful applicant will be a good generalist GP, understanding Primary care, and will also develop enhanced skills in elderly care and acute medicine (including for example point of care diagnostics).

The role will be to help with the comprehensive assessment of complex and potentially unstable housebound patients, providing support for care planning and helping put in place interventions to enable the patient to recover at home, or to reduce the need for hospitalisation. Diagnosis, decision making and team working are key. The service will build up a caseload of patients who will be clinically managed on behalf of their GP practices for an agreed period at home until stable.
