With the NHS Long term plan, Ageing Well agenda and current transformation of community services, it is an exciting time to be working in frailty services and older people's medicine.

At Frimley Health we are an integrated acute and community services provider and with Ageing Well funding, will see expansion of our community services and older people's medicine directorate over the coming years. This new post has arisen due to the need for clinical input into a new intermediate care facility in Bracknell, called Heathlands, which is due to open in February 2022.

Main duties of the job

  • Be responsible for the medical care of 20 patients receiving rehabilitation at Heathlands intermediate care unit, working alongside a junior doctor, advanced nurse practitioners and therapists.
  • Work across our community services based at Farnham hospital and there is also the opportunity to undertake a clinic and work with our newly established hospital at home service.
  • Commit to appraisal, revalidation and personal development in line with statutory and Trust guidelines.
  • Assist in developing the service and participate in departmental audit and quality improvement projects.

About us

The successful candidate will be joining a team of eight geriatricians and cross cover will be provided for annual leave. There is flexibility in the job plan to work around the successful candidate's subspecialty interests

Qualifications
Essential

  • MRCP
  • 10 years experience in relevant medical specialties
  • Full GMC
  • Awareness of legal responsibilities eg safeguarding, mental capacity act, deprivation of liberty, data protection, equality and diversity

Desirable

  • Higher degree eg MSc, postgraduate diploma

Specific Competencies
Essential

  • Able to demonstrate innovative thinking
  • Demonstrate appropriate leadership behaviour, including the ability to organise and develop service provision
  • Demonstrates ability to work effectively in multidisciplinary and interprofessional teams.

Professional Experience
Essential

  • Clinical experience of general management principles in community medicine and rehabilitation and interface with community services and social care
  • Able to undertake comprehensive geriatric assessment.
  • Able to independently manage an outpatient clinic
  • Ability to lead MDT meeting and experience of effective discharge planning.
  • Previous experience in delivering interprofessional and post graduate teaching
  • Understand principles and participated in audit activity and quality improvement
  • Understand the principles of Clinical Governance and demonstrate experience of evidence based practice

Desirable

  • To have completed a teaching or educational supervisors course
  • Previous participation in MRCP &/or student teaching
  • Subspecialty experience eg movement disorders

  • Able to demonstrate leadership qualities
