This is a newly created post, and the successful applicant will join the dynamic and motivated consultant physician/geriatricians and multidisciplinary teams with a special interest in frailty.

The services and provisions for older people have been prioritised, invested in, and made a strategic priority for the Trust; therefore, the successful candidate is joining at a really exciting time for the frailty and older peoples services, and will be key in supporting the innovative and exciting developments including the implementation of frailty assessment in SDEC, inpatient admission pathways, community pathways, virtual ward and sub-acute management, and short-stay frailty.

The post-holder will have a middle grade doctor, an F2/CT and an F1 as part of their team. 

The Trust has 3 Frailty Practitioners established in post, with further posts to be advertised, who will assist with the management of patients presenting with and requiring specialist frailty care across the variety of settings and environments in which the frailty team function.
