About us

A great opportunity has arisen to join our vibrant rural GP practice as we sadly say goodbye shortly to one of our GP Partners who is retiring. 

Working from 3 sites - Four Crosses and Llanfyllin being the main hubs of care- we provide a high level of patient care and have a strong track record of developing services locally for our very supportive patients.  We are a training practice at all levels from Doctors to receptionists and have a large multidisciplinary team. 

Allowing people to develop their ideas and build on clinical strengths is a priority for us as well as having someone who respects the vital part that every member of the primary care team brings to effective patient care. 

Frequent and varied social events in our stunning rural area which is easily accessible to towns and cities as well as an ethos of mutual support in difficult times add to all of our ability to cope with the day-to-day stresses that is the NHS today.

Who is the team? 

There are 7 GP partners, 2 Salaried GPs and a regular locum as needed. Most of us work part time and all have special interests (rather than special needs!) 

We always work together and there always someone in the next room. We get on well and generally pass ideas between us and thrash out clinical and non-clinical problems in a non-judgemental manner where everyone’s opinion counts. 

We have a prescribing Pharmacist who provides medical expertise, supports our training of dispensary staff and mentors Pharmacy interns. Tony is the first point of call after discharge, after being seen in clinic and for acute medication queries

We have 5 prescribing Nurse Practitioners who support same day services as well as a Paramedic who undertakes visits and supports the holistic care we provide for our frail elderly 

We have 5 Practice Nurses who provide chronic disease management, routine nursing care, immunisations etc.

We have 3 HCAs who are constantly interested in developing their work and knowledge.

We dispense to 90% of our 11000 population which requires a large dispensary team of a high skill level, headed up by our Dispensary Manager Elise

The clinical team could not function without the non-clinical team which is headed up by our amazing Practice Manager who brings a transparent and open approach to all aspects of our large team’s activities both day to day and in planning for the future. 

No problems appear too large or insurmountable indeed it is made to seem easy. Juliet works closely with everyone but particularly her management team- Finance Manager, Operations Lead, Pharmacist, IT and data lead with strong admin support.

We have a large reception and admin team and really could not manage without their signposting for patients as well as recognition when a patient is struggling and needs more help than is being asked for. 

We are lucky to have our District Nurse team on site in Llanfyllin and have a weekly 1-2 hour MDT with our Social Worker, community therapies team, elderly dementia team, voluntary sector member, Macmillan nurse to discuss elderly frail patients, cancer patients and patients of concern as well as a monthly meeting with the health visitor and school nurse and drug and rehab services. 

So how do we work? 

Working in a rural area means we are the one stop shop for our patients from the minor trivial to the chest pain, fallen off a ladder type problem. We cover what on paper seems a very large area but does not appear so when you are here. It is sometimes rather a relief to be out visiting as the journeys are beautiful. 

We work from 3 purpose-built sites all with their own idiosyncrasies- mainly Llanfyllin and Four Crosses as GPs but we fight over who goes to Llanrhaeadr as it is so beautiful and quiet.

So, a patient rings as they need a doctor- the receptionist works out if they need 999, if they need someone that day or need a routine appointment with a GP or a nurse on another day.

For patients who need someone that day a few details are taken, and the patient’s name is put on the list for the duty team. This consists or a GP in Llanfyllin and a GP in Four crosses with a nurse practitioner on each site and our paramedic working between. Everyone picks names off the list according to their strengths and calls the patient, works out what is needed and may call the patient in or arrange a visit if needed or manages the case over the phone. The GP works in the same way but also provides mentorship and support for the ANP or Paramedic where needed. 

We encourage discussions and learning as that provides support. We have the ability to order Xray’s locally that day if needed and arrange blood tests with results that day up to 5pm.  We also provide a minor injuries service for cuts, bumps etc. For the acutely unwell patient whom we support while awaiting an ambulance , having our Paramedic is a great resource.

Patients who need a GP routinely for a new problem are asked a few details. If it is a skin problem, they are booked directly into our skin clinic which runs weekly. Otherwise, the patient is phoned back within 24 hours by one of our nurses. This allows us to make sure it is not urgent, not a suspected cancer, arrange any blood tests that may be needed, signpost to the right person if needed and makes sure that the GP appointment is both timely and enough time is allocated to allow a thorough focussed and useful appointment. 

In house we take all bloods for ourselves and secondary care, undertake ECGs, 24-hour ECGs, event monitoring and week long ECGs. We can arrange US, Xray’s, MRIs and Ct scans locally or in Shrewsbury the DGH.

We provide minor surgery, IUD fitting and implants, diabetic specialist service with insulin initiation led by our diabetic lead nurse and GP. We provide all chronic disease management, immunisations, child health surveillance, smears, we have good relations with the primary care mental health team for counselling and good links to the community mental health team. We are hoping to have a Physiotherapist on site soon to support musculoskeletal service which are very common particularly in our farming community.

As a dispensing practice, the patient can be spoken to, diagnosed, seen and have their medication within hours.

We know our patients very well as many of us live in the community. This gives a depth and colour to our care, and we provide care at end of life to the majority of our patients where it is a natural and expected event either at home, or in our local care homes. This is only possible with our close working relationship with the district nurses and wider community teams and because of our team approach from Practice Manager to the very valued cleaners. 

Training and development 

We are a forward-thinking training practice and currently offer training placements for GP Registrars, Medical Students, Physicians Associates, Nurses, Paramedics and Pre-Registration Pharmacists along with having developed bespoke training plans for our reception, dispensary, and administration teams. 

We provide a half day training session each month for all our team members to allow time to meet as a team, have a structured clinical meeting and protected training time for all team members.

We are now a greener practice and this is just one of the many areas we have piloted over the years. 

Locally 

We have a very supportive patient population who not only regularly donate to support what we can offer but actually throughout COVID remained staunchly positive about us and what we were doing; understanding when we had no staff and the presents of cakes, tea, chocolate, wine was as appreciated as the many heartfelt comments.

Our area, set in North Powys, Mid Wales yet close to the Shropshire border, offers wonderful walks, along with quiet country lanes for riding and cycling (but even our champion bike rider needs a car for visits) . The stunning Welsh Coast, the mountain ranges of Snowdonia, the Berwyns and beautiful Lake Vyrnwy are all easily accessible.

We have excellent local schools but some of the GPs live in Shrewsbury and Wrexham and there are excellent accessible private schools. Despite looking rural it is easy and quick to get to local towns and cities as well as to the coast and mountains, while a cow or two might hold you up briefly queues are very rare and usually commented on in Facebook.

Too good to be true?

Working in the NHS nowadays is inevitably stressful and with all the arrangements in the world things can happen. What we make sure of is that no one is ever dealing with too much alone - if the days goes pear-shaped, we all step in to help whether that’s helping another Doctor, a nurse, or the Pharmacist. 

If something is not right or something goes wrong for a patient, we look at it together, try and work out what happened and then work out what we think might be better or what we all need to learn. Nobody comes to work to make a mistake and there for the grace of God go all of us and that is how we approach problems. 

On the more positive side we enjoy making things work better for patients both as their advocates in secondary care, working with our local health board for funding to improve what we can do here and developing all our skills and interests, so we stay sane and enjoying what we do as much as possible. 

We like getting together; we like partying; our yoga class run by our ANP kept the senior partner sane through COVID. Lunchtime walks are a regular and evening paddleboarding and canoeing are very amusing. Bike trips and hill walking helped burn off the calories after the Macmillan cake making fundraising was rather too successful. 

There are a couple of good practice parties in the year but many more informal evenings across and within the teams which are widely shared (and often laughed at). Our children and their friends support the practice and get good training in throwing a party and cocktail making among other vital skills.
