Job Details

We are looking for dynamic and proactive GPs with a passion to improve patient care for the post of Salaried GP in our Health Board Managed Practices. SBUHB currently have one managed practice split across two sites in the Port Talbot area of the Health Board. The post holder will be required to work across both sites in Cwmavon and Cymmer to deliver quality, safe and consistent care to our patient population. The post holder will join a well established MDT of GPs, Practice Nurses, Pharmacist, Medical Assistants and administrators who drive service improvement and innovation in accordance with key Health Board and Welsh Government priorities.

The post holder will be required to navigate multiple digital platforms including Ask My GP, Vision and Microsoft 360 as part of their daily role. They will be expected to lead in specific areas of improvement to ensure current evidence based practice remains the focus of our service delivery. Leadership opportunities will be available to those who wish to pursue professional development within the Health Board.

Main duties of the job

The postholder will undertake a senior role within a Multi-disciplinary team that provides care to patients registered at Cymmer/Cwmafan Health Centre. They will ensure the sustainability of general medical services through the delivery of patient-centred, safe, effective primary healthcare services and provide direct clinical input to meet patient demand and expectation through new ways of working. Normal duties, workload and important non-clinical roles will all be articulated through a Job Plan and there is a requirement to work flexibly.

Job description
Job responsibilities

The detailed duties of the post holder are as follows:-
  • To provide appropriate care and services to the patient population of Cymmer and Cwmafan for all age ranges and type of health care needs, working to a high quality and with other team members
  • To respond to medical problems presented by patients, including history taking, examination and investigation where appropriate, diagnosis, treatment and referral as needed
  • Ensuring that adequate and appropriate timely clinical records are maintained for all manual and computer systems
  • To be part of a dynamic and adaptable clinical team that delivers both proactive and reactive care as appropriate as and when necessary.
  • To conduct clinical practice in accordance with contractual requirements within the parameters of the SBUHB organisational policies and procedures and ensure local/national guidelines are adhered to.
  • To provide management of long term conditions in line with national guidelines and pathways
  • To prescribe in line with SBUHB Medicines Management guidelines and protocols to ensure safe prescribing and prudent use of resources
  • Regularly review medications and monitor repeat prescribing
  • To provide appropriate health promotion and preventative health care advice to all patients
  • To initiate an MDT approach to patient care where appropriate to ensure a holistic and prudent approach to patient care
  • To undertake on-call duties as part of the practice on-call rota, where agreed. This may include requesting that patients present at the surgery, telephone consultations, house visits and triage
  • To provide medical information for the development of systems appropriate for the SBUHB needs.
  • To participate in departmental staff meetings and contribute to policy/protocol creation and implementation
  • To attend other departmental, Locality and Health Board meetings as necessary in support of or represent the Managed Practice in place of the Clinical Director or Group Medical Director
  • Support the in-depth evaluation of current service delivery and collaboration with service teams and external stakeholders to determine appropriate development pathways.
  • Additional expenses of any kind will not be committed without the approval of the appropriate manager/budget holder
  • To be familiar with and actively promote adherence to the regulatory framework including NICE and National Service Framework Guidance
  • To undertake all work in accordance with the SBUHB procedures and operating policies
  • Adhere to SBUHB prescribing guidelines
  • To contribute actively to the content areas of the 1000 Lives Campaign and any other subsequent patient safety campaign
  • To adhere to the SBUHB incident reporting protocols and utilise the DATIX reporting system where appropriate
  • To respond in a timely manner to all requests in the investigation of a complaint and act in accordance with policy outlined in the Putting Things Right document
  • To undertake regular audit of clinical services within your area of responsibility to ensure continuous improvement and service development
  • Observe and maintain strict confidentiality of personal information relating to patients and staff
  • Be responsible, with management support, for own personal development and to actively contribute to the development of colleagues
  • To ensure you remain current and updated with all aspects of clinical practice associated with this job description and individual job plan
  • To develop and/or maintain teaching and management skills in line associated with this job description to support service delivery and MDT development
  • To participate in an annual Job Planning Review process
  • To participate in the Annual GP Appraisal process
  • Be responsible, with management support, for own personal development and to actively contribute to the development of colleagues
  • To facilitate learning conditions for improved training opportunities in line with national and local recommendations
  • Where agreed, to act as a Mentor to wider MDT members for further education courses and encourage the development of extended skills to enhance service delivery.
  • To provide appropriate health education and advice to patients to encourage and enable self-management and alternative healthcare access choices in future
  • To undertake appropriate training to meet personal and mandatory educational needs and service development needs

Qualifications
Essential
  • Full GMC Registration with a license to practice and entry on the GP Register
  • Included on a Welsh Medical Performers List
  • Evidence of continuous professional development
  • Knowledge of National Agendas in respect of Primary Care
Desirable
  • Higher Post Graduate Membership e.g. MRCGP, MRCP etc
Experience
Essential
  • Experience of working as a General Practitioner
  • Experience of and/or willingness to provide mentoring and support to alternative Clinicians
  • Extensive multi disciplinary working
Apptitude and Abilities

  • Demonstrable skills in written and spoken English adequate to enable effective communication about medical topics with patients and colleagues
  • Proven ability to handle a busy and varied primary care based caseload and respond flexibly to workload fluctuations
  • Ability to take independent clinical decisions when necessary and to seek advice from senior colleagues as appropriate
  • Commitment to team approach and multi-disciplinary working
  • Effective counselling and communication skills
  • Understanding of clinical risk management and clinical governance
