This role is for a Consultant Psychiatrist to join an Adult Mental Health South Durham Community Affective Disorders Team and Access Service based in Spennymoor, County Durham.
Main duties of the job
The Consultant will be expected to provide committed, proactive and responsive clinical leadership working with other staff to provide an assessment and treatment service for those individuals suffering with Affective Disorder in the South Durham Locality.

We provide a range inpatient and community services to 2m people living in County Durham, the Tees Valley, Scarborough, Whitby, Ryedale, Harrogate, Hambleton and Richmondshire. We also provide additional specialist services to other parts of Northern England.
We deliver our services by working in partnership with eight local authorities and clinical commissioning groups, a wide range of voluntary organisations, as well as service users, their carers and the public. The services are spread over a wide geographical area of around 3,600 square miles, which includes coastal, rural and industrial areas.

The South Durham (Sedgefield) Affective team has approximately 580 service users on its caseload. The team receives approximately 90 referrals per month which is into Access service. The number of patients coming into the Affective Team per month varies with an average of 15. We continually work towards reducing numbers through ADUP process and following this Pathway lends itself to endings and subsequently discharges. The medic workload within the team is determined by MDT decisions in Daily Decision Meetings and Formulations. This process ensures the team psychiatrist is not overloaded with work that can be undertaken by other staff members.
The new post-holder's workload will comprise a small personal caseload of 10-15 comprised of patients who are on CPA (Care coordinated by other team members). On average over the last three years there has been no more than one CTO for the team at
one time. The Consultant Psychiatrist will be a member of the South Durham (Sedgefield) Affective Leadership Team (Supercell) working in close partnership with the other Team Psychiatrist, Team Manager, Team Psychologists and Advanced Practitioner in a variety of leadership activities. This includes attending leadership time-out sessions held across the locality, which involves meeting with other leadership teams to provide a consistent leadership presence across community services. The team operates in keeping with the Purposeful and Productive Community teams Trust wide Project. Therefore, the team consultant is also required to be involved in communication huddles, service user formulations, and a range of multi-disciplinary working activities. There will be one day of access duty per week. Access to specialist services is available.
Core and Higher Specialty Trainees from the local psychiatry training schemes rotate into adult mental health. Trainees from the Tees Valley and Northumbria Vocational Training Schemes join the Trust for 6 months psychiatry experience every February and August. Foundation Programme Year 2 doctors also come to the Trust every 4 months to gain experience in psychiatry.

The nature of our community services have changed significantly over the last three years due to New Ways of Working in the Community and the reorganisation of Inpatient Services. As well as providing patient review clinics, the Consultant will also undertake joint reviews with other team members for complex, unstable and high risk individuals. They will be fully involved with the CPA process. They will be expected to provide clinical support and supervision to the team in conjunction with the Team Manager, Team Psychologists and Advanced Practitioners. They will attend team meetings and the leadership team meeting with Team Manager and Advanced Nurse Practitioner on a weekly basis. There is no inpatient commitment, but close liaison with In-patient Consultant colleagues is expected regarding individual service users if admitted to an in-patient bed. The team works within a Bio-Psycho-Social Recovery-Focussed model. The post-holder will carry a very small personal caseload of 10-15 patients - the work will be done in conjunction with other Team Members. On average the post-holder will see 4 new (access) patients for week and 10 follow up patients and perform a day time on call 9am to 5pm once a week and as part of it will be required to undertake Mental Health Act Assessments if happens while on call during day time.
There are operational criteria and referral protocols for the Affective Disorder Teams. The team works closely with the Psychosis Team. The Team is able to obtain expert advice from Trust-wide specialist services which include an ADHD service; the Autism Spectrum Assessment Team and the Eating Disorder service. The Trust has access to the Regional Affective Disorders Service in Newcastle. The post-holder will need to develop effective working relationships with personnel in these services.
Patients are referred to the service directly from General Practitioners and other primary care mental health services, for example the IAPT (Improving Access to Psychological Therapies) service. Thus, the Access service acts as a 'gatekeeper' to secondary care services.
New patients are initially seen by the Access Service, which has a central hub and spoke structure with close liaison with Access nurses co-located at the team base. A newly developed Access Plus service provides short term interventions up to 12 weeks, and local general practices are also served by GP aligned mental health nurses with whom the postholder is expected to develop good working relationships.
Our Crisis Team manages access to in-patient services. The post-holder will need to develop effective working relationships with members of the Crisis Team and In-patient Team by closely liaising with these services when patients are referred to the crisis team / needing alternative to hospital beds will be managed by the crisis team and the patients who need to be in hospital will be managed by the inpatient team.
The Trust provides specialist services for substance misuse problems. Alcohol services are commissioned separately.
The post holder will be expected to provide appropriate supervision to junior medical staff and non-medical prescribers within the team. The 3 non-medical prescribers within the team who are Senior clinicians with experience in prescribing but may need some support and supervision.
Supervision responsibilities are as follows:
0.6 Speciality doctor who is Section 12 approved (currently vacant)
1.0 CT educational/weekly supervision by this post holder
If the post holder is a new Consultant they will be supported to develop into a trainer role after the first year.
The post holder will be expected to have familiarity with all important, relevant legislation affecting the Adult Mental Health population.
Daytime on call once per week 9am-5pm for Mental Health Act assessments. On these days, routine patient work is kept to a minimum and Mental Health Act work is generally carried out in the afternoon.
The post holder will participate in the Consultant on-call rota for adult mental health services (2 acute inpatient wards, 20 beds each at Lanchester Road Hospital, Durham) and MHSOP for North Durham (Bowes Lyon Unit, Lanchester Road Hospital Durham) which is currently a 1:15 and receives a Category A on-call supplement of 3%. Senior Registrars will take part in the tiered on call rota but the frequency will vary depending upon the number of senior registrars in the locality. Liaison psychiatry team operates 24/7. The on-call psychiatrist provides cover for the acute hospital, police station assessments and section 136 assessments.

- MB BS or equivalent medical qualification. Fully registered with the GMC with a licence to practice at the time of appointment. Included on the GMC Specialist Register OR within 6 months of gaining CCT in General Adult Psychiatry
