About the team:

The Forensic Outreach Service (FOS) is a specialist, multi-disciplinary service operating within the Forensic Mental Health Services to facilitate and manage the safe and efficient transition of high risk adults with mental disorders from secure forensic settings (medium and low secure care) into the community, and eventually, back into their appropriate Community Mental Health Team or Community Forensic Team.

Transition from secure services to the community is recognised as a particularly difficult time. FOS provides ongoing mental health assessment, formulation and treatment to promote recovery and manage risk during and after transition between services in order to safely facilitate patient’s eventual acceptance back to their local teams, if appropriate.
