This is a new combined Paediatric and Adult Emergency Medicine specific post (7DCC 3 SPA) to support the development of Emergency Medicine at SBUHB , part of a consultant recruitment plan to enhance the current workforce of 13 wte consultants leading the Emergency Department at Morriston Hospital. Following a major workforce review the Health board is committed to enhancing the senior team further to allow a gold standard, consultant delivered service, supported by robust middle and junior decision maker tiers. Our workforce plan has been developed to provide quality patient care delivered by a team supported to reach their full potential; enhance their skills with dedicated time to develop special interests and with innovative job planning and progression for both sustainable working and a quality work-life balance.

Main duties of the job

We are keen to support applications from colleagues with recognized subspecialty training in PEM, gained via the RCEM route (or demonstrable equivalent training). We would expect the successful applicant to join the enthusiastic but fledgling Paediatric Emergency Medicine team, whilst also maintaining their skills in Adult Emergency Medicine. We recognise the value that colleagues with shared training offer to the department, with colleagues working formal 50:50 consultant posts split between ED and Intensive care, as well as colleagues who already work a mix of adult and dedicated paediatric ED shifts.

Like many smaller departments we struggle to provide the quality of care we aspire to, or to meet the 2019 RCPCH standards for Children in Emergency care settings. In response to these challenges we have the support of the Health Board Executive Team to develop a single point of access for all paediatric unscheduled care attendances. We continue to work with the acute paediatric team and primary care colleagues to develop a robust process, workforce plan and business case for this service. This change would increase our attendances to around 25,000 per year, and we are on the lookout for colleagues with experience in service improvement to help provide, shape and improve the future of care for children and young people. Supporting this development as well as leading ongoing service improvement initiatives would be a key part of this role.

About us

The Emergency Department at Morriston Hospital is the major unit for South West Wales seeing more than 80,000 new patients per year, of which approximately 20,000 are children and young people. The unit benefits from the on-site support of all major specialties with ophthalmology, neonatology and obstetrics & gynaecology located on the Singleton hospital site in Swansea and neurosurgery and paediatric intensive care provided at University Hospital of Wales in Cardiff.

Job responsibilities

You will provide a clinical service, with colleagues, which will include the responsibility for the prevention, diagnosis and treatment of illness. Working with consultant colleagues and the multi-professional team you will have continuing responsibility for the well-being of all patients under your care; allowing for proper delegation to, and training of staff. In partnership with clinical and managerial colleagues you will also have responsibility for the safe, efficient and effective functioning of the services in which you work in line with the values, operational policies and integrated medium term plan (IMTP) of the Health Board. You are encouraged to consider how services can be improved and to report any concerns about safety.

You will be required to adhere to the principles of good medical practice as laid down by the General Medical Council.

You will be required to participate in the Health Boards risk management process, clinical governance and any other healthcare governance activities within the department, the Service Group and the Health Board. Adherence to the Caldicott Principles of information governance is mandatory.

You will be required to maintain a license to practice through revalidation, to undergo satisfactory annual appraisal using the all Wales online Medical Appraisal Scheme (MARS) and to formulate a Personal Development Plan, in conjunction with the Clinical lead, to identify training and development needs. You will be expected to participate in sufficient personal and professional development to fulfil Royal College and GMC requirements. You will also be required to undergo a job plan review at least annually and to agree and then meet service outcomes.

You will be expected to make a significant contribution to the service and Directorate planning process through active participation in the clinical Service Group structure.

At present the dedicated PEM consultant rota consists of a mixture of day and late shifts, with a minimum of one late shift per week 1:6 weekends but currently no-on call. The adult ED rota consists day, middle and late shifts, on-call every third week and 1:6 weekends of which every alternate is on call. This mixed post would continue with a 1:6 weekend frequency and at least one late shift a week, however the adult ED on-call frequency would be halved. Applicants need to be aware that as we continue to develop both adult and paediatric services the rota and on-call commitments may change. Bank holidays are shared equitably between consultants. All out of hours work attracts enhanced hours equivalents.

The consultant contract in Wales has a 7 DCC, 3 SPA split. Clinical administration is included within DCCs, 1 SPA is for Personal Development, the other two support trainees and additional activities, as evidenced in job planning and appraisal.

You will adhere to all policies which cover radiation exposures as currently outlined in IRMER regulations.

The Health Board is committed to meet its obligations to minimize infection. You will be required to comply with current procedures for the control of infection, including dress code, to challenge non-compliance by colleagues, and to attend training in infection control provided by the Health Board.

Qualifications
Essential

  • Registered with the GMC
  • Licensed to practice
  • APLS or EPLS provider
  • ATLS or ETC provider

Desirable

  • On Specialist Register with GMC with sub-speciality training in Paediatric Emergency Medicine, either via the RCPCH or RCEM training program, or demonstrable equivalent training
  • Eligible for CCT within 6 months of date of interview and have passed exit exam
  • Instructor in APLS / EPLS / ATLS / NLS
  • Teaching qualification/Higher qualification in medical education
