This is an exciting opportunity to be a pivotal part of an integrated team supporting the patients with Long COVID.

Our integrated post covid team has now been established for approx. 12 months and works with our Occupational therapy, physiotherapy, clinical psychology and talking therapist. We work closely with colleagues in the acute to support the small number of patients needing specialist input. We wish to ensure that those needing our on going support do so are signposted to the most appropriate clinician and supported in the most the appropriate setting,, whether face to face or virtual.

The post holder(s) would ideally have an interest in long covid and working within an integrated way supporting the wider Frimley health system. We would encourage applications from those interested in quality improvement who would like to see a truly integrated approach for our local population.

We currently have 4 vacant weekday sessions (currently Tuesday and Thursday) these can be divided amongst successful applicants.

Main duties of the job

To include:

  • Coordination of patient care and clinical input within the MDT
  • Review referrals for the week's MDT if sent beforehand by members of MDT
  • Take part in clinical supervision or triage of referrals on a rota system
  • Chair MDT or other meetings relating to service as appropriate
  • Respond to medical queries flagged by the post-Covid MDT, care coordinator(s) and suggest if medical interventions are required, and escalate areas of concern for consultant input where needed via email or phone/MS teams
  • Discuss complex patients and see virtually or face to face according to local arrangements where necessary

Job description
Job responsibilities

Clinical/Managerial Duties

 

  • Coordination of patient care and clinical input within the MDT
  • Review referrals for the weeks MDT if sent beforehand by members of MDT
  • Take part in clinical supervision or triage of referrals via ERS as needed on rota system
  • Chair MDT or other meetings relating to service as appropriate
  • Respond to medical queries flagged by the post-Covid MDT, care coordinator(s) and suggest if medical interventions are required, and escalate areas of concern for consultant input where needed via email or phone/MS teams
  • Joint responsibility with therapists for MDT discussion and input into development of MDT care plans
  • Action log will be created following MDT and assign responsible clinicians
  • Discuss complex patients and see virtually or face to face according to local arrangements where necessary
  • Provide clinical input for Children and Young People Transitioning to Adult Services where required
  • Complete onwards referrals to Specialist Clinics, Community Services e.g. social prescribers as required and follow up telephone calls as required
  • Signpost to vocational rehabilitation services and med3 certification/fitness for work as required
  • Overview of onwards communications, ensuring all information is cascaded to patients own GP and other relevant parties
  • Contribute to the smooth running and administration of the MDT where necessary
  • Engage in regular CPD relating to Post Covid 19 Syndrome not limited to but including NHSE webinars, Future NHS Collaboration platform resources, in house training

 

  • To share the learning with the wider system.
  • To develop positive collaborative working relationships across all areas of the care team including community.
  • To work collaboratively with the Consultant and the entire team to ensure that effective clinical governance is incorporated in the day to day practice.
  • To provide appropriate health promotion and preventative health care advice to all patients attending Frimley Health NHS Foundation Trust (FHFT).
  • To maintain clinical records either written or computer based.
  • To work with colleagues to identify ways to ease the burden of demand.
  • To work in line with appropriate internal and external policies and procedures including human resources, safeguarding children etc.
  • To provide appropriate health education and advice to patients and carers to enable self-management and choice.
  • To share and disseminate acquired knowledge with all frailty team members through developmental sessions and clinical governance meetings or critical incident reviews
  • To develop and maintain clear communication with patients, carers, families and colleagues, ensuring effective care delivery.
  • To promote effective communication processes between all team members and community colleagues to enable effective collaborative working practices.
