Due to the expansion of our Specialist Community Forensic Team we are currently looking for a substantive Consultant Community Forensic Psychiatrist to join our team. This is a 10 PA post and you will be based at various placements across the county. We are actively considering full time working over 4 days to enhance the working lives of our consultants, and we are also open to applications for people looking for part remote working.

Main duties of the job

You will join an extensive, close working multi-displinary team, providing high quality, psychologically informed, risk managed care for a cohort of patients with complex forensic mental health needs.

You will have the option to opt out participating in the on call rota, and you will have 1.5 PA’s plus up to 1.0 additional SPA for undertaking a specific role, such as; medical appraisal, governance group, service development, research, academic and others in line with you interests, career aspirations and Trust objectives.

Detailed job description and main responsibilities

An exciting opportunity has arisen for the post of substantive Consultant Community Forensic Psychiatrist with our expansion of our Specialist Community Forensic Team.

This is a full-time post based at various placements across the county. The service is actively considering full time working over four days to enhance the working lives of consultants. We will also consider partly remote working again to ensure the wellbeing of our consultants. The post-holder will have an office, access to a laptop and mobile. The successful applicant will also have access to admin support which may be shared with another Consultant.  The consultant will work with a full multi-disciplinary team and provide clinical leadership input.

Somerset NHS Foundation Trust has resulted from the merger of the former Somerset Partnership NHS Foundation Trust (SomPar), the Mental Health Trust in the area, with Taunton & Somerset NHS Foundation Trust (TST), the Acute Trust, on 4 April 2020.  The merger was the first of this kind in the country. The post holder will be able to participate in the development of the merged Trust and help bring to fruition the benefits to our mental health patients the merger will provide. There are now plans in place for the further expansion of Somerset NHS Foundation Trust by merging with the Yeovil District Hospital.

The Service

The Forensic Service provides interventions for patients in Somerset who experience mental health difficulties and are in contact with the criminal justice system, or who present with significant risks to others. Over time, the team has developed and now comprises a number of services including:

  • Specialist forensic locality leads, supporting Somerset CMHT’s, inpatient settings and external stake holders with patients presenting with a forensic concern.
  • A Liaison and Diversion service based in Bridgwater police custody, covering the Somerset area of Avon and Somerset Constabulary (ASCC)
  • An OPD (Offenders with Personality Difficulties) service for the National Probation Service
  • IIRMS ‘Reach’ service for the resettlement and support of offenders after custody
  • Assertive Case Management for Somerset service users placed in secure settings outside the county
  • MAPPA responsibility for the Trust
  • A new Specialist Community Forensic Team.

The Specialist Community Forensic Team (SCFT) is an exciting, innovative, newly commissioned service to Somerset FT, working inline with the South West Provider Collaborative.

The team will be responsible for overseeing the care of Somerset patients who are currently detained to a secure hospital within the South West region.

The aim of the SCFT is to reduce the length of patient stay for patients in secure hospital and work to prevent admission to secure care by

  • Providing enhanced community support, to facilitate, support and maintain discharge processes.
  • Providing clear, tiered levels of intervention with the aim of admission avoidance, including risk management.
  • Gatekeeping admission requests alongside Ash ward (Somerset FT Low secure ward).
  • Encouraging proactive discharge discussions for longer stay patients who may have faced barriers to the discharge process.
  • Supporting community placements.
  • Establishing and maintaining good working links with stakeholders including, the SWPC, NHS and Private sector inpatient secure care providers, MAPPA, Ministry of Justice and the wider trust.

The advertised role will form part of an extensive, close working multidisciplinary team, who will provide high quality, psychologically informed, risk managed care for a cohort of patients with complex forensic mental health needs.

Currently in Somerset there is already a Low Secure ward, Ash ward, led by Dr John Barnes. Ash ward is based in Bridgwater. There is also a PICU ward, Holford Ward, led by Dr Nathan Maynard. It is expected that the post holder will be working closely with both consultants.

There are also close working relationships established with Fromeside Medium Secure Services in Bristol and Pathfinder Services, as well as the developing Forensic services in Devon.
