An exciting opportunity has arisen for  a part time General Practitioner to join our highly motivated, personable and patient focused Complex Care Clinical team to deliver the best care to our patients in Yeovil and the area to the West of Yeovil.

We would welcome enquiries or applications from existing and newly trained General Practitioners, and are flexible on the number of sessions.  The salary is being reviewed to ensure it remains competitive and attractive.

Main duties of the job

A key focus of our Complex Care team is providing good quality patient centred and patient specific care looking at their medical, functional, mental and social circumstances.  This means care is largely delivered in patient’s homes and is achieved through good links with community partners, through either informal links and more formally by attending community MDTs to support care for patients.

We are determined that our patients receive the best possible experience and outcomes and by constantly challenging ourselves to put the patient at the heart of everything that we do.  We are achieving this by working in innovative ways with partners such as hospital specialist teams, GP colleagues, social workers, Rapid Response, Neighbourhood Teams, Village Agents and others.
