Here at Somerset NHS Foundation Trust we are in the exciting position to be expanding our innovative Palliative Medicine Consultant service. We are looking to appoint a colleague who will play a central role in the delivery of a visionary single palliative medicine service.

This is a great opportunity for a consultant to join Somerset NHS FT and play an integral role in the planning and delivery of future services. A role in which education and development of others is so important it is built into the job plan from day one.  Palliative Medicine Consultants in Somerset enjoy a breadth of experience in community and inpatient care. The post holder will have opportunity to shape their post over time to practice within a community, inpatient or outpatient basis as part of the consultant team.

We invest in our colleagues and support one to one clinical supervision for consultants and have a robust mentoring system for consultants new to the trust.

Our service serves a catchment population of approximately 430,000 which reflects the current St Margaret's Hospice footprint. There is an expectation of partnership working with other hospices, including Dorothy House and Weston Hospicecare in serving those respective population. Patients are empowered earlier in their journey and are cared for in the place of their choice.

Main Duties of the Job:

  • This post will provide medical consultant  support to the YDH palliative care team alongside the palliative care nurse consultant in YDH and clinical support for the community palliative care team in the East of the county.
  • Encourage a holistic response to the care of patients and their families.
  • Develop Somerset palliative care services in line with appropriate guidelines and frameworks.
  • Support Somerset End of Life individualised care planning.
  • Deliver  palliative and end of life education to a wide range of health professionals and public.
  • Part of the consultant team providing senior medical palliative care clinical services to patients across a variety of settings including in the acute trust, community, outpatients, community hospitals and hospice.
  • Undertake out-of-hours responsibilities, including participation in on-call cover on a 1 in 8 remote ‘2nd on call’ basis.
  • If out of hours work has included attending a unit, time in lieu will be considered to facilitate work life balance.

Somerset NHS Foundation Trust is the first NHS Trust on the English mainland to provide community, mental health and acute hospital services. We work with health and social care partners in Somerset to ensure that we deliver outstanding services that meet the needs of our population.

Our trust was formed on 1 April 2020 when Somerset Partnership NHS Foundation Trust and Taunton and Somerset NHS Foundation Trust merged.  The end of life care services in the county are well positioned for the potential future merger of YDH and Somerset FT, as a step in the development of the ICS.

Location - Yeovil:

YDH is a modern hospital in pleasant countryside only two hours from London and less than one hour from the coast.  Yeovil is a large town with a good range of independent shops, arts and leisure facilities and schools.  Surrounded by the beautiful countryside and villages of South Somerset, it is also close to the coast and has good transport access to larger towns.
