Are you a consultant with a strong interest in Community Care of the Elderly, Acute Frailty, Dementia, Palliative Care? Are you an energetic person who has a clear vision of the future direction of clinical services, an interest in quality improvement and clinical governance?

This exciting new role will support our ongoing comprehensive reconfiguration of services for unscheduled care which involves further development of our Community Hospitals.

This is an RCP approved post

Main Duties of the Role

Community Hospitals services operate throughout four sites of the Trust with the step down and rehabilitation wards on all four sites at Gainsborough, Louth, Skegness and Spalding. The directorate is currently in the process of developing and strengthening its stroke and frailty pathways, ensuring patients are provided with great care, close to home.

The community division has intermediate care beds on our four hospital sites and are looking to strengthen links with Integrated Neighbourhood teams.

Applicants should hold full registration with the General Medical Council.
