We’re recruiting a team of geriatricians in east Kent to take forward a dynamic and innovative new programme dedicated to keeping older people with frailty out of hospital and at home for longer.

We’re all about innovation. During the pandemic we provided hospital-level treatment for nearly 300 people at home, including people with covid-19, with oxygen and dexamethasone. We’re already working in a multi-professional way with over 25 Advanced Clinical Practitioners in the combined acute and community frailty teams working to support frail older people in east Kent.

We work in acute and community hospitals, in partnership with care homes and primary care, and with the local voluntary sector to support people with frailty to stay out of hospital, if that is their wish.

Main duties of the job

To lead and develop integrated care working with CCGs, acute trust and community partners.

Be an advocate for people living with frailty within the organisations.

Lead and develop interface frailty services for relevant clinical areas.

Lead, support and mentor junior staff (medical and allied health professionals) within the department and ensure training requirements are met.

Undertake clinical work within a relevant area of interest.

Build a strong reputation and culture in east Kent of a new and innovative way of providing care to our frail population.

Detailed job description and main responsibilities

Frailty services are changing in east Kent to deliver integrated care across boundaries, in line with the needs of our local population.

These new posts are an exciting opportunity to shape innovative and high quality, patient-centred services in a sustainable way.

We want to move away from institutionally-based care and breakdown organisational boundaries with a view to providing care as close to home as possible in line with what patients say they want.

The ongoing pandemic has demonstrated our commitment to delivering rapid change for the benefit of our frail population in east Kent. The British Geriatric Society identified 10 areas of innovation during the coronavirus pandemic, of which four were examples from east Kent.
