The Vacancy

You will provide a comprehensive community psychiatric service to the South of Lincolnshire (comprising Spalding, Holbeach, Crowland and Market Deeping) for those over the age of 65 with dementia and functional psychiatric disorders including patients with early onset dementia.

You will be an integral member of the community mental health Team (CMHT) based at Johnsons Community Hospital, Spalding and will not be responsible for any inpatients.

There is a full-time Specialty Doctor attached to the team as well as a very experienced multidisciplinary team comprising CPN, OT’s, Social Workers, Assistant Practitioners and Admin staff.

As the post holder you will have admitting rights to all Older Adult Acute Inpatient Units within the Trust, but all care and treatment for patients admitted to these units will be provided by a separate dedicated inpatient multidisciplinary team.

The total catchment area population of those aged 65 years and over is approximately 28,000 and the team receives on average 16 referrals a week, circa 65 per month equating to 785 annually. All referrals go through a Single Point of Access (SPA) for triage and allocation to the most appropriate part of the service. Referrals received from SPA are then allocated to appropriate non-medical members of the team for initial assessment and if required, the medical members (including the Consultant) are then consulted for advice on further management of patients.

In line with new ways of working, the Consultant is not responsible for all the cases referred to the team but provides input and support to the Care Co-ordinator and other members of the MDT when required. It is therefore anticipated that the consultant will have a very small and dynamic case load.

With the increasing use of technology in the organisation, a significant amount of work is now carried out remotely via secure online platforms. This includes case reviews and consultations, OPC, HVs, MDT reviews and liaison with the PCNs and GPs. There is also access to a wide range of relevant clinical information from primary and secondary care teams within an online care portal. As a Consultant within the Team you will therefore be encouraged to work in a consultative way using technology to support your day to day practice.
