We are looking for enthusiastic and motivated Consultants in Geriatric Medicine to join our team to support our developing Elderly Care and frailty service and be a part of shaping and delivering these exciting developments. We are in the process of reorganising the way we deliver safe and effective care to older frail patients across the whole healthcare system in North Hampshire.

Covid-19 has affected all our services but our restoration and future resurgence planning are underway. It is an even more important time to advocate for our patient group and deliver sustainable covid secure services.

Our Service includes a front door frailty team, consisting of a Consultant Nurse, a clinical matron, frailty specialist practitioners, senior doctors, a pharmacist and a service co-ordinator. We aim to assess patients presenting to ED with a multidisciplinary approach and plan a timely, safe discharge following full medical assessment. For those patients who require an in-patient admission we are currently developing our frailty liaison service to provide ongoing specialist support to our downstream wards. We also have responsibility of downstream Elderly Care wards for those patients that are admitted. The wards run daily board rounds and are supported by Clinical Matrons and allied health professionals to provide a multidisciplinary team approach to patient care.

We offer rapid assessment clinics throughout the week, as well as weekly Falls clinics and Parkinson’s clinics. The TIA and Stroke service is provided by the specialist team based in Winchester.

We work closely with the OIder Persons Mental Health liaison team and we are developing our links with the local Primary Care Networks to improve integrated care for older people in our area.

We support flexible working patterns where that is required.

On-call commitments are on the acute medicine rota. They currently involve a medical admitting take approximately 1:14. (Additional 2PAs for on call duties) There is a weekend ward round rota approximately 1:12 for morning ward round seeing targeted patients on the medical wards with time off in lieu. This might be negotiable for applicants who are not currently practising GIM.

The applicant should hold a CCT in General Internal Medicine and Geriatric Medicine or be within 6 months of completion.
