Anticipated Start Date: ASAP

Closing Date for Applications: Midnight on 12th November 2020

This substantive consultant post will be based in Abergavenny alongside 5 Consultant Geriatricians (with specialist interest in stroke, orthogeriatrics, nutrition and Parkinsons disease) working within Nevill Hall Hospital. The department is designed and supported with a team of; Clinical Nurse Specialists, Non Training and Training grade doctors, physician associates, Therapists and Pharmacists. The team prides itself on providing high quality Acute Medical care and comprehensive geriatric assessments for the elderly, which is consultant led with good multi-disciplinary working.

With a large elderly population we have seen a constant demand to expand our service. The planned developments will see improved partnership working with our primary care and community partners to ensure the Elderly are treated at home or in the most appropriate place. There is close joint working with Frailty teams and front end geriatrics. Our services will be developed using best practice principals with regard to acute medical care of older people.

The potential candidate will take responsibility for Elderly care/ rehabilitation/ EFU wards and will also have out-patient clinics and on-call commitments for unselected admissions.

The Clinical Futures Care of The Elderly model will improve the patient experience, so the ideal candidate can have a key role to play in shaping this service joining our existing team for elderly services comprising experienced geriatricians, discharge co-ordinators, nursing staff, and rehab staff all of whom are dedicated to provide good quality patient care. The role of geriatricians and other professionals who specialise in care of older people is crucial in implementing best practice in terms of leadership, education and training.
