It is an exciting time for ELFT working with a new expanding Primary Care team. The Trust provides mental health, primary care and community health services across East London, Luton and Bedfordshire. In 2020 we have developed a dedicated primary care directorate that reflects our commitment to providing primary care services that benefit the populations we serve. As a large outstanding rated Trust there are great opportunities within ELFT to develop innovative and diverse careers.
Our vision is to improve the lives of all we serve by providing truly integrated care. We have an opportunity at The Newham Transitional Practice to make a real difference. The team play an active role in the local Stratford PCN and have good links with other system partners. The practice provides specialist services to the local homeless population, and in partnership with our other primary care services in East London has been involved in the provision of a new outreach service. The outreach service was developed in response to the Covid-19 pandemic and provides primary care services to rough sleepers who have been moved into hotel accommodation. The outreach service is evolving so that it can meet the needs of the homeless population in Newham, Tower Hamlets and Hackney.
This role would involve 7 sessions at Newham Transitional Practice and 2 sessions as part of the primary care outreach team. We are seeking a talented and committed GP to work within the team to provide care in innovative ways that will really make a difference to the lives of the people we serve.
This post would offer:
9 sessions per week (8 clinical sessions and 1 QI session)
