      The selection of relevant exposure groups in any study design needs special consideration as this may weaken the interpretation of the results. The RAMQ analysis is contemporary with the publication of the first asthma guidelines [20] and of the increase acceptability of ICS treatment for asthma. While ICS are the cornerstone of asthma management, they remain, as monotherapy agents, not approved or recommended for COPD while ICS/LABA combinations were only approved for use in COPD in Canada in 2003 and therefore were not reimbursed for COPD in the last few years of the analysis in the RAMQ.
