Although a selection of rapid responses will be included online and in print as readers' letters, their first appearance online means that they are published articles. The NHS, paid for through taxation, with the purchasing might of the NHS, is incredibly efficient (despite changes such as the purchaser-provider split and the health and social care act, which increase transaction costs, and FPI and outsourcing to for-profit organisations).
If anybody thinks the NHS is unaffordable - try private health insurance. Especially the sort of system in the USA, where many people simply cannot afford healthcare.
