Healthcare cost is increasing and with new technology more and more patients will seek help. When healthcare is free at the point of delivery like our NHS, it is the role of those leaders to make sure every penny is spent properly and is good value for money.

As such, the NHS is wasting a lot of money due to poor systems, poor process, silos working, poor efficiency, DNAR, cancelled lists and lack of well trained staff, rota gaps and poor investment in IT in secondary care for many decades.

The NHS is also not very safe! It is good value for the money we spend! These technology investments will be one more burden and when patients are over diagnosed there will be an increase in demand to provide healthcare and we will get in a vicious circle of wasting more money!

The NHS needs more money for transformation, and there is no question about it. But it also needs to stop wasting money by looking at value for money and good leadership and good governance. It also needs to tackle club culture, old boys' network and efficiency and throughput and productivity of individual consultants and the team. The good news is most consultants work very hard and are willing to work hard if there are good systems and processes, theatres are run smoothly and there are well trained adequate staff.

We all need the safest and the best NHS. Criticising the NHS is not going to solve our problem. The time is right for doctors to take on a leadership role in our wonderful NHS and to transform it and stop working in silos and invest in IT and good digital health and make it the safest, the best and most vibrant health and social care and for that we need transformational leaders.

There must be digital transformation, cultural transformation, leadership transformation and also service transformation, with health and social care being transformed together. Without this the NHS will not survive for too long and without the NHS our nation will not be strong and vibrant. It is in our best interests and the time has come for good doctors to take on leadership roles and work with senior NHS leaders and managers and transform it and make it one of the safest and the best health and social care service in the world.
