Darzi calls for overhaul of safety incident reporting

The systems for reporting safety incidents in the NHS need to be overhauled, according to two reports led by former health minister Ari Darzi.1 2

The report Patient Safety 2030 said that as little as 5% of patient safety incidents are reported. It calls on healthcare providers to cultivate an open and transparent culture, as staff are often put off reporting errors for fear of being blamed.
