1 Have discharge coordinators

With winter comes a rise in demand for healthcare, which is mainly due to respiratory conditions leading to a spike in emergency admissions. Guidance from the UK National Institute for Health and Care Excellence (NICE) on how to avoid delays in discharge recommends a single health or social care practitioner to coordinate each patient’s discharge plan, including information about the patient’s drugs.1
