NICE recommendation on the use of pressure ulcer risk scores is flawed

The updated National Institute for Health and Care Excellence guideline on preventing pressure ulcers claims to reflect current evidence.1 However, its recommendation on assessing pressure ulcer risk is surely not evidence based.
