Time for open access to all medical documentation forms

Currently, most of the documentation forms that lie behind clinical trials and electronic health records are not accessible to the public. The next step towards sharing data from clinical trials1 is for these forms to be made freely available, and then rationalised. As these are blank forms, issues of data protection don’t arise.
