The article correctly identifies the difficulty of dealing with the increasing number of patients with multiple morbidity.

The suggested solution of allowing patients to directly book appointments with a doctor negates the value of triage, allowing the patient to be seen by the most appropriate professional and saving doctor time. Emails from patients when I am not at the practice worries me. This is not a 'fail safe' system; we know that patients are poor assessors of the significance of their symptoms and doctors do go on holiday.
