In reply to Scott and colleagues,1 we found an unequivocally wide variation in reoperation after colorectal surgery. Furthermore, variation is identifiable at high surgical caseloads, suggesting that centralisation of surgical services, a policy commonly applied to reduce variation, may be ineffective.
