All 8500 GP practices in England are to receive a charter pledging the standard care that patients will receive at the end of life.
The charter, drawn up jointly by the Royal College of General Practitioners (RCGP) and the Royal College of Nursing (RCN) with input from specialists, patient groups and others in health and social care, sets out seven pledges to patients.
